SC Registration Form & Documents

All Service Centre (SC) must fill up the application form and submit the relevant documents prior to starting. All documents are to be submitted to the recruiter or agent. Agent then will need to submit to EM for verification and filing.

SC Application Form

Registering SC as a LIMITED company 

1. Photocopy of Certificate of Incorporation
2. Photocopy of Certified Memorandum and Articles of Association
3. Photograph of proposed location of operation/ Proof of business location

Registering SC as a Sole Proprietors

1. Photocopy of Certificate of Registration
2. Photograph of proposed location of operation/ Proof of business location 
3. Photocopy of Valid Identification (I.D.).(carry copy of the original ID i.e. Passport, Driver’s License or Work permit)
